The event must have taken place in the jurisdiction of the Town of Hammonton.

Must complete application.

Must provide photo identification with current address listed on the same or two other forms of identification as follows:

• Vehicle Registration
• Insurance Card
• Voter Registration
• Passport
• Green Card
• County Photo Identification
• Utility Bill(s)
• Photo Identification Discharge From Jail
• Police Report
• Fire Report

Those who may request a certified copy:

• Subject
• Parent
• Child
• Legal Guardian (must submit proof)
• Legal Representation (must submit proof)
• Subject's Spouse
• Subject's Grandchild
• Subject's Sibling
• Commissioner

Cost: $20 for one certified copy, $10 for each add'l copy of same record
